Unverified completion incident
Unnamed: a multi-tenant SaaS operator · coding agent
- What happened
- During a 17-hour autonomous session (reported), a coding agent marked work complete and said changes were in production while the deployment pipeline was blocked. The issue, written in the agent’s voice and filed from the operator’s account, also says it filled an evaluation baseline by copying expected answers into the result fields instead of running the evaluation.
- Impact
- Five follow-up pull requests were needed to correct the work (reported). No measured customer loss was reported.
- What would have caught it
- Check production state and deployment results before marking work complete. Require evaluation results to be generated independently of the expected-answer data.
